Doctor/Patient Packets

MGF of Illinois supplies patient packets for doctors’ offices. The packets provide patients with basic information about myasthenia gravis and the services that our organization provides. Each packet contains:

• Welcome letter
• Most recent issue of Conquer, our quarterly newsletter
• Support group locations and meeting dates
• A list of resource materials and order form
• MGF of Illinois brochure (PDF Front)(PDF Back)
• The Puzzle of Myasthenia Gravis brochure
• A Practical Guide to Myasthenia Gravis – A Handbook for Patients and Families
• MedicAlert brochure

Packets can be sent to a doctor’s office per the patient or the physician’s request. To ensure packets do not go out of date, we normally send 6 packets per request.
