
This award honors Tracy Shackelford, an important and compassionate member of the MG community who passed away in 2021 after a long battle with myasthenia gravis. This award is created in her name to recognize others who demonstrate a high level of service to the MG community. The recipient of the Tracy Shackelford MG Service Award will be announced at the Snowflake Shuffle event. Eligibility:
- Any individual who lives in Illinois, Indiana or Wisconsin can be nominated.
- Persons being nominated for this award will have consistently demonstrated qualities of compassion, and service to the myasthenia gravis community.
- Nominees serving as a Conquer MG board member, or a committee member can be considered.
- The nomination application should focus on volunteer work that benefits the MG community.
Judging Criteria:
- The degree to which their services/actions have made a lasting or meaningful contribution or impact on the MG community.
- The length and degree of their service(s) will be considered.
- The extent to which their service or action might be considered above and beyond the call of duty.
- The extent to which their action would be recognized and valued by their peers/community leaders.
Presentation:
- The winner of the Tracy Shackelford MG Service Award will be announced at the Snowflake Shuffle for MG this year on Sunday, June 7. The winner does not have to be present to win.

- Full name
- Contact phone and email address
- Mailing address (only individuals who live in our service area are eligible)
- A 500-word explanation about what this person has done. Include information about why you think it is meaningful or has a lasting impact on the MG community.
